Simply send us an extended residence certificate /household composition issued by your municipality of residence, or a “déclaration sur l’honneur” (for residents in France) via our myCMCM app, by e-mail to affiliation@cmcm.lu, or by post to: CMCM – L-2971 Luxembourg.
